2012 - 2013 Athletic Participation Information
| The following forms are REQUIRED of all athletes and students working with the athletic teams prior to the start of the sports season. Physicals are valid for the entire academic year after June 1st, 2012 and should be submitted to the athletic office as soon as completed. We recommend keeping a photocopy of the physical at home. |

PIAA Comprehensive Physical Form - complete and submit to athletic office. This is the only form acceptable to meet the PIAA requirements. Athlete Emergency Information – complete and submit to coach. Parent/Guardian Recertification - must be completed before participation in a 2nd sports season and again for a 3rd sports season PIAA Medical Recertification - must be completed by the treating physician for injured athlete to return to subsequent sport participation Please read the Insurance Information and Code of Conduct prior to signing the Athlete Emergency Information form. |
